Description
Yolo
Community Care
Continuum (YCCC) is a non-profit agency
that provides an array of community-based programs for people experiencing
a psychiatric disability. YCCC has provided services in Yolo County for
over twenty eight years. Each service is designed to meet the individual
needs at specific points in the recovery process. Twenty-four hour supervision
is offered in three residential programs: crisis and two long-term. The
representative payee program provides money management support for consumers'
finances while being in compliance with all State and Federal guidelines.
Supportive housing offers an opportunity to live more independently in
the community.
Each YCCC
program encourages increased levels of independent living. All programs
are based in community settings. Clients receive support from staff, peers,
and volunteers. Client input is actively solicited and utilized in program
evaluation, program planning, and agency-wide planning.
A person
must be a client of Yolo
County Department of Alcohol, Drug and Mental Health Services (Farmhouse
and Safe Harbor programs), Placer
County Department of Mental Health (Harmony House program), or with
a county that has a current contract with YCCC to access YCCC programs.
A person may enter two or three programs simultaneously. To ensure continuity
of care, treatment efforts are coordinated between programs, among other
public and private providers, as well as with family members.
Admission
and participation in the programs are the same for everyone without regard
to race, color, sex, creed, or handicap.
