Haven House Interim Care Program is a short-term medical respite care facility that offers homeless individuals a quiet and secure environment to focus on improving their health.
The stable, shared-living environment at Haven House allows individuals to obtain outpatient services that would otherwise be unavailable to them, such as physical therapy, occupational therapy, or home health care.
Additionally, individuals receive transportation to health-related appointments and are linked to community resources to assist them upon release from the program.
Referrals to Haven House are made only by Partnership and Yolo County HHSA. Admissions last for six weeks or less and are available only to those who are experiencing both homelessness and a health-related incident that requires supplementary care away from the hospital.
Individuals must be willing to comply with doctor’s orders, be ambulatory, and be able to manage their own daily living activities (such as preparation of provided food). Individuals must also agree to strive for improved well-being, meet with support staff, and maintain a safe environment.
